HOW IT WORKS
Step 1: You Send Us The Referral
Once you’re a client, discharge tracking requests are submitted through this site using a secure, client-only link. You’ll also receive your unique submission link by email after signing up. Please allow a couple of hours for our system to refresh and provision access. If you need the link sooner or can’t locate it, you can always email [email protected].We begin tracking as soon as the request is received, even if discharge timing is still uncertain.
Step 2: We Track and Follow up
We monitor the expected discharge window and follow up directly with the case manager as timing changes. No one on your team has to remember, chase updates, or revisit the same referral repeatedly.
Step 3: You Get a Clear Outcome
Your intake team is notified when the patient is actually ready for discharge or when the referral is explicitly delayed or closed so every referral ends with a clear resolution.
